Internal regulations

1. GENERAL PROVISIONS

1.1 These Rules define the behavior of patients and other visitors, as well as the terms and conditions for the provision of paid dental services (hereinafter referred to as "Dental Services") at "Doctor Danich" LLC (hereinafter referred to as the "Clinic" or "Medical Center").

1.2. These Rules are displayed for general acquaintance at the clinic's reception in a freely accessible location.

1.3. The Clinic provides specialized outpatient dental care.

1.4. The Clinic offers paid medical services in dentistry based on a license issued by the Ministry of Health of Ukraine.

1.5. Primary, pre-doctor, doctor-level, and specialized medical care includes the following services:

(a) Primary pre-doctor care: Radiology, nursing, dentistry.

(b) Specialized medical care: Therapeutic, surgical, orthopedic dentistry, and orthodontics.

1.6. The price list for the dental services provided by the Clinic, specifying the types and costs of services, is available at the reception in a freely accessible location.

2. RULES OF PATIENT BEHAVIOR AT THE CLINIC

2.1. Patients (Visitors) must wear shoe covers upon entering the Clinic and leave outerwear in the cloakroom.

2.2. Patients may enter treatment rooms only upon invitation and under the supervision of Clinic staff.

2.3. Minors under 14 years old are allowed in the Clinic only when accompanied by close relatives or guardians.

2.4. Accompanying persons may stay in the treatment room only with the doctor’s permission and must follow their instructions.

2.5. Dental records are the property of the Clinic and are kept at the Clinic’s reception.

2.6. Patients are obligated to:

(a) Be respectful and tactful in their interactions with medical staff.

(b) Avoid visiting the doctor under the influence of alcohol, drugs, or other substances.

(c) Appear for appointments on time and notify the Clinic in case of inability to attend.

(d) Respect the rights of other patients and Clinic staff.

(e) Follow the Clinic's procedures and public behavior rules.

(f) Avoid interfering with the doctor’s work or the medical assistance process.

(g) Show respect to other patients and Clinic staff.

(h) Treat the Clinic’s property with care and maintain cleanliness and quiet in the premises.

2.7. To ensure public order, prevent offenses, and guarantee the safety of staff, patients, visitors, it is prohibited to:

(a) Bring firearms, gas or cold weapons, toxic, radioactive, chemical, explosive substances, alcohol, or any items that may pose a threat.

(b) Be in service areas without authorization.

(c) Take out of the premises of the Clinic the documents received for review.

(d) Remove documents or medical records from the Clinic.

(e) Leave advertisements or other materials without management approval.

(f) Take photos or videos without prior permission from management.

(g) Engage in commercial or unauthorized activities on Clinic premises.

(h) Smoke inside the Clinic or on the attached territory.

2.8. Individuals under the influence of alcohol, drugs, or displaying aggressive behavior are not allowed in the Clinic. Such individuals will be removed from the premises by Clinic staff or law enforcement.

2.9. Violating these rules may result in remarks or measures according to applicable laws.

2.10. Obstruction of medical services, disrespect toward staff or patients, or disturbance of public order may result in administrative liability as per Ukrainian law.

2.11. Doctors have the right to refuse service if the patient's behavior violates these Rules, provided it does not threaten the patient’s life.

3. TERMS AND CONDITIONS FOR PAID MEDICAL SERVICES

3.1. Patients can obtain information about services, prices, and payment procedures at the Clinic’s reception.

3.2. Services are provided by appointment, which can be scheduled in person or by phone.

3.3. New patients are scheduled for an initial examination and consultation with an available doctor. Specific doctor requests are accommodated based on their schedule.

3.4. Patients arriving more than 20 minutes late may be rescheduled to avoid delays for subsequent patients.

The Clinic is not responsible for the inability to notify the patient about appointment changes if the provided contact details are incorrect, the phone is disconnected, or the patient fails to answer calls.

3.5. In case of unforeseen circumstances, the Clinic will notify patients promptly using the provided contact details.

3.6. The Clinic sends appointment reminders to patients to confirm attendance and optimize scheduling. The Clinic is not responsible for being unable to reach the patient due to incorrect contact information, a disabled phone, or unanswered calls.

3.7. If a patient cannot attend their scheduled appointment, they must notify the Clinic in advance.

3.8. In certain cases, the Clinic provides dental services based on a written agreement with the patient or an informed consent form for specific treatments (hereinafter referred to as the ‘Agreement’).

3.9. The Clinic informs patients that failing to follow medical advice, including treatment regimens, may reduce the quality of the services provided, delay completion, or adversely affect the patient's health.

3.10. Patients are seen by doctors with appropriate specialization and qualifications. Information about the doctors is available at the Clinic’s reception.

3.11. The Clinic may delay services until the patient signs the Agreement or consent form.

3.12. The Clinic may refuse services if: The patient has contraindications or conditions that make treatment unsafe; The patient refuses necessary diagnostic examinations; The patient insists on unsafe treatment methods; The patient is under the influence of alcohol or drugs.

3.13. Each patient’s medical history is documented in a Dental Medical Record, which is stored at the Clinic and includes examination results, treatment plans, and medical interventions.

3.14. The Clinic provides guarantees for specific dental services, as outlined in the Agreement. Guarantees apply exclusively to the relationship between the Clinic and the patient and may be voided if the Agreement terms are violated.

4. PATIENT RIGHTS AND OBLIGATIONS

4.1. Patient Rights:

4.1.1. Respectful and humane treatment from staff.

4.1.2. Choice of doctor.

4.1.3. Preventive, diagnostic, and therapeutic services in sanitary conditions.

4.1.4. Access to specialists' consultations.

4.1.5. Relief from pain associated with illness or procedures.

4.1.6. Protection of medical confidentiality.

4.1.7. Compensation for harm caused by medical services, within the limits of the service cost.

4.1.8. Refusal of medical intervention, with an explanation of potential consequences.

4.1.9. Access to information about their rights, health status, and treatment options.

4.1.10. Copies or extracts from medical records upon written request.

4.2. Patient Obligations:

4.2.1. Take care of maintaining their health.

4.2.2. Follow the treatment regimen, rules of behavior in the Clinic, and norms of conduct in public places.

4.2.3. Arrive on time for appointments and notify in advance if unable to attend due to a valid reason.

4.2.4. Be present for treatment and examinations at the agreed time set by the doctor.

4.2.5. Follow oral hygiene practices and carry out preventive measures recommended by the doctor.

4.2.6. Follow the doctor's recommendations when undergoing various treatment methods.

4.2.7. Inform the doctor of all necessary information for diagnosing and treating the disease.

4.2.8. Inform about past illnesses, known allergies, contraindications, and provide other information that may affect the quality of treatment.

4.2.9. Strictly follow all prescriptions and recommendations from the doctor.

4.2.10. Immediately inform the doctor of any changes in health status during diagnosis and treatment.

4.3. The patient is obligated to pay for the medical services provided by the Clinic within the terms and conditions specified in the Agreement.

V. RIGHTS AND OBLIGATIONS OF MEDICAL STAFF

5.1. The treating doctor (physician) has the right to:

5.1.1. Independently determine the scope of research and necessary actions aimed at establishing an accurate diagnosis (diagnosis) and providing medical assistance (treatment).

5.1.2. Require preparatory procedures for professional oral hygiene (removal of plaque and tartar) before treatment begins.

5.1.3. With notification to the patient, make changes to the treatment (treatment plan) and carry out additional specialized treatment.

5.1.4. Refuse to continue treating a patient if the patient violates the treatment regimen and timelines, does not follow the doctor's treatment instructions (recommendations), or exhibits rude, disrespectful behavior.

5.1.5. Defend their professional honor and dignity.

5.1.6. Improve their professional knowledge.

5.2. The treating doctor (physician) is obligated to:

5.2.1. Provide medical services in accordance with the requirements for diagnostic, preventive, and treatment methods for diseases of the dental system (teeth, oral cavity, temporomandibular joints), within the framework of the agreed treatment plan and medical indications, and in compliance with the legislation of Ukraine.

5.2.2. Based on the examination results, provide the patient or their legal representative with accurate information about the patient's health status in a clear and accessible form, including information about examination results, presence of disease, diagnosis, prognosis, treatment methods, associated risks, possible medical intervention options, their consequences, the nature of the proposed treatment, and expected treatment outcomes.

5.2.3. Provide the patient with information about the progress of medical services in a clear and accessible form, about possible complications and discomfort during and after treatment, including those related to the patient's individual characteristics, and about the prescriptions and recommendations that must be followed to maintain the achieved treatment results.

5.2.4. Not disclose information that constitutes medical confidentiality.

5.3. The nurse is obligated to:

5.3.1. Ensure patient safety by adhering to aseptic and antiseptic rules, as well as maintaining sanitary requirements for medical institutions in accordance with the legislation of Ukraine.

5.3.2. Accelerate the doctor's work for patient reception by assisting, preparing the workspace and equipment for the reception, and helping with filling out medical documents.

5.3.3. Carry out orders from the medical director, director, and administrators of the medical center.

5.3.4. Not disclose information that constitutes medical confidentiality.

6. RIGHTS AND OBLIGATIONS OF THE CLINIC

6.1. The clinic is obligated to provide the patient with the following information by placing it on its website and on the informational board:

(a) the name of the legal entity;

(b) the address of the legal entity's location, the document confirming the entry of the legal entity's details into the registry, specifying the registration authority;

(c) information about the medical activity license, the date of the decision to issue it, and the list of services;

(d) a list of paid medical services with prices in the national currency of Ukraine;

(e) the clinic's working hours, the schedule of medical staff (doctors) involved in providing paid medical services;

6.2. In case of an unforeseen absence of a doctor on the day appointed for medical services, the clinic has the right to assign another doctor or cancel the appointment, taking measures to notify the patient in advance.

6.3. After the full execution of the paid medical services contract, upon the request of the patient or their legal representative, the clinic provides the patient (or legal representative) with medical documents (copies of medical records, extracts from medical documents) reflecting the patient's health status after receiving the paid medical services.

6.4. In its relations with patients, the clinic applies all possible measures to resolve disputes and contentious situations by mutual agreement with the patient.

6.5. Complaints and claims from patients should be presented orally directly to the clinic's director during designated hours or in writing.

6.6. During the review of complaints or claims, the clinic has the right to request additional information from the patient that is relevant for addressing the complaint or claim substantively.

6.7. The clinic does not accept claims from patients regarding the quality of medical care and health damage caused by private opinions of specialists from other medical organizations that are not authorized in accordance with the law to conduct an expert evaluation of the quality of medical care.

6.8. The clinic is not responsible for delays in providing services or for deficiencies in the provided dental service if they occurred:

6.8.1. Due to the patient's non-compliance with the doctor's instructions and recommendations, including failure to follow, incompletely follow the doctor's referrals for diagnostic examinations to clarify or exclude the diagnosis of the disease; failure to follow recommendations for consulting a related specialist; failure to attend or timely attend an appointment with the doctor for treatment or continuation of ongoing treatment; failure to follow recommendations regarding the intake of medications and their regimen; failure to follow recommendations for treating detected dental and related diseases, and non-compliance with the agreed treatment plan;

6.8.2. Due to the patient's non-compliance with the rules for using the results of the provided service, including orthopedic constructions, restored teeth, etc.;

6.8.3. Due to actions of third parties or force majeure.

6.9. The administration has the right to demand compensation from the patient, according to the price list, for not attending the clinic without informing the doctor at least 6 hours before the appointment.

7. LIABILITY AND DISPUTE RESOLUTION

7.1. Patients’ claims about the quality of medical care must be based on authorized expert evaluations, not personal opinions or third-party assessments.

7.2. The Clinic is not liable for delays or deficiencies caused by: non-compliance with medical advice or prescribed treatment, improper use of dental work, such as prosthetics, external factors or force majeure events.

7.3. Patients may be charged a penalty for missing appointments without notifying the Clinic at least six hours in advance.

8. CONTROL AND SUPERVISION

8.1. Control over compliance with the rules for providing paid medical services by healthcare organizations is carried out by:

The Ministry of Health of Ukraine.

The Main Directorate of the State Service of Ukraine on Food Safety and Consumer Protection in Dnipropetrovsk Oblast.

9. ADDITIONAL PROVISIONS

9.1. These Rules are mandatory for all patients and visitors of the Clinic.

9.2. Amendments to the Rules are made by the administration of the Clinic and take effect upon their publication in publicly accessible areas of the Clinic and/or on its official website.

9.3. By using the Clinic’s services, patients confirm their agreement with these Rules and commit to complying with them.

9.4. For issues not covered by these Rules, the Clinic operates in accordance with applicable laws of Ukraine.
